Our process

01 New Patient Exam

$175

45 minute appointment either in-person or over the phone. Includes
intake, review of health history and past labs, x-rays if in-person,
and testing recommendations unique to your needs and concerns.

02 Testing

$200 - 400 Per Test

Including a variety of testing as recommended by your provider such as bloodwork, stool testing, urine hormone testing, & more! If you are not local to our clinic, we can ship testing to you to be done locally or at home depending on the type of testing.

03 Test Read/ Interpretation:

$300

After all of your test results have come back, you will meet with your provider to review them. This also includes a recommended care plan by your provider, any necessary supplementation, & lifestyle changes.

04 Supplements

$20-100 each

Your provider will recommend supplements specific to your lab results. We utilize our own brand of supplements that can be purchased at our clinic or shipped to you.

05 Ongoing Care Plan

Your provider will meet with you to follow-up with and monitor your progress, adjust supplements, provide lifestyle modification recommendations, as well as recommend any further testing. Two options: 1- Pay As You Go starting @ $375/consult 2 – Discounted Pre-Pay starting @ $275/consult [10% discount on supplements for term of pre-paid]

APPOINTMENT CANCELLATION POLICY:

Appointments may be cancelled or rescheduled for no charge 24 hours prior to original appointment time. Appointments cancelled, no-showed, or rescheduled less than 24 hours prior to appointment time will endure the following charges: $50

PRICING INFORMATION

SERVICES NOT BILLABLE TO INSURANCE

Insurance companies do not allow any of the above services to be billed.
If you have an HSA (Health Savings Account)/ FSA (Flex Spending Account) they may or may not allow for reimbursement. Please check with your HSA/FSA to determine what they allow to be purchased with HSA/FSA money.
Consultation and nutrition pricing is based on the individual. Once test results are received, a clinic member will meet with you to determine approximate pricing of your care plan and offer discounted pricing on bundled services. (Example plans on back of this sheet) Contact the clinic for more information.

DIAGNOSTIC TESTING (LAB WORK)

SERVICES THAT MAY BE BILLABLE TO INSURANCE

Please note that just because a service is billable to insurance does not mean it will be covered by your insurance provider.
Chiropractic care plans are available at a discounted rate when insurance is not being billed. (Sample on the back of this sheet)
Some of the services may be billable to Medicare and other state insurance. They may only cover services for providers located in the state your insurance is through.
